Job Description

We are looking for AFC Caregivers, Epic Nursing Services is an AFC Intermediary authorized by the Massachusetts Executive Office of Health and Human Services (“EOHHS”) to participate in MassHealth’s Adult Foster Care (“AFC”) program.

QUALIFICATIONS.

  • is at least 18 years of age with the ability to make mature and accurate judgments and with no mental, physical, or other impairments that would interfere with the adequate performance of the duties and responsibilities of an AFC caregiver; 
  • Does not suffer from alcohol or substance use disorder; is able to devote appropriate time necessary to provide needed personal care to the Participant to ensure the member’s safety and well-being at all times; and
  • is not a legally responsible relative of the Participant.
  •  Pursuant to the AFC Program Regulations, the AFC Caregiver is responsible for providing the Participant with nutritionally balanced meals at least three (3) times daily, including snacks and fluids as appropriate to meet the Participant’s needs, and that respect the Participant’s dietary restrictions as well as ethnic or religious preferences.

RESPONSIBILITIES OF POSITION

i.    supervision of the Participant;
ii.    supervising and assisting with Activities of Daily Living and Instrumental Activities of Daily Living (as those terms are defined in section 408.402 of the AFC Program Regulations) of a Participant that is necessary for the Participant’s health and well-being;
iii.    monitoring and reporting any non-urgent or nonemergency changes in the Participant’s medical condition to the AFC Intermediary. In cases of emergency, the AFC caregiver must report directly to the most appropriate provider and follow up with the AFC Intermediary;
iv.    maintaining the home environment shared by AFC Caregiver and Participant.
v.    completing the caregiver log required by AFC Program Regulations and sending it at the end of each month to the program’s registered nurse;
vi.    issuing reminders to the Participant about prescribed medications and timely refilling the Participant’s prescriptions;
vii.    assisting with or arranging for Participant transportation to medical and other appointments;
viii.    assisting the Participant to comply with health-care instructions from health-care providers;
ix.    promptly arranging for medical care that the Participant needs; and
x.    notifying the AFC provider of the need for alternative care of the Participant
